6 weeks after surgery

My breasts are me...they are a part of me and feel as if they have always been there. 
I can do anything now, in terms of lifting and exercise, but I know I have to start off slow and steady.
I am about to go overseas to visit my family and can’t wait until they see me.
I bought myself a new bikini and the lady in the shop didn’t even notice.
She just went...”Wow, You look amazing in that.”
I had to tell her that I had my breasts done and she said she would never have known and she has been working in swimwear for 2o years.
She couldn’t believe how natural they looked and how perfectly proportioned they were.
This has been the best thing I have ever done for myself.
Having kids with the man I love has been the best thing I have ever done, but having my breasts done has been the best thing I have ever done for me!
I would not have done it differently, nor would I have gone to anyone else. 
Dr. Rastogi is a genius! I feel like my life is different now, better...
I am a much happier person than I was before. I was always happy and cheerful with other people, but I used to go home and think...”I wish I could be as positive about myself, as I am with other people:”

  • Wear sun protection every day. Cosmetic physician Dr Geoffrey Heber says, "It has been shown if you do nothing else but use a sunscreen, it allows the body's natural repair processes to prevent and repair skin damage over time."

IPL

Diary from my patient's Breast Augmentation journey

Sydney-based Make-up Artist and Hair Stylist, Jessica Berg has been using her expertise to boost women’s confidence for over a decade. Little did I know, that she, herself, had developed low self-esteem as after breast-feeding her children she was left with a hollowed out chest. Who would have thought that this beautiful, willowy former model lacked confidence!
I suggested, that perhaps she should seek some advice in regards to a breast augmentation and recommended she have a consultation with cosmetic surgeon, Dr Anoop Rastogi who specializes in the procedure.
Here is her story (in her own words) on how she gained back her self-confidence and femininity…
I have always wanted to have boobs! I never really had much shape at all. Before I had my two beautiful boys, my cup size was a very small B.
When I fell pregnant for the first time, eight years ago, it was an amazing boost to my ego, because I got some much wanted shape to my all over frame.
Being pregnant made me feel feminine for the first time in my life.
I was lucky to have plenty of milk and chose to fully breast feed both boys, without supplementation.
I breast-fed my eldest son for 15 months and my second son for 12 months. Although I wouldn’t change a thing it definitely took a toll on my whole body, particularly my breasts.
Having gone from a full C cup whilst breast-feeding, to what I have now – a double A has made me very self-conscious about my breasts. I won’t leave the house without wearing a bra, not even to get the newspaper. Sometimes I will even wear a bra to bed, as I don’t feel comfortable.
My husband is a great support and says he doesn’t mind my breasts the way they are, but I do mind. I feel shapeless if I’m not wearing one of my many heavily padded bras.
Sometimes, I’ll try and make light of it by joking that I need bigger drawers to fit all of my bras in.
So after lots of research and sole searching I made the decision to have a breast augmentation, as I truly wanted to feel happy and confident about my body.
I also wanted my boys to see a happier, more confident mummy. I owe it to them to give them the best of myself.

Love the skin you're in…..


Here are some of the ingredients to look for in skincare, remembering that many of the really good ingredients have several functions
Vitamin A, C and E: are powerful antioxidants which shield your skin from oxidative damage. ( a major cause of skin aging)
Retinols: are excellent at repairing fine lines and wrinkles, improving orderly metabolism and cell communication and stimulating new collagen and elastin. However they can be irritating so you need the right ones.
Alpha and Beta Hydroxy acids (AHA, BHA): these include ascorbic acids, Salicylic acid lactic acid, glycolic acids etc great for exfoliating the skin, improving pigmentation, promoting smoothness, stimulating blood flow and improving collagen content.
Niacinamide (Vitamin B3):  is a superstar newcomer which is excellent at improving pigmentation by preventing melanin uptake into skin cells. It produces both skin clarity and translucency. It is also a powerful antioxidant and improves cell to cell communication, so is very versatile.

The Verdict on Macrolane

Why did you decide to have the procedure?
Since having children I have been very unhappy with my breasts but didn't want to have surgery. I had this treatment to fill the emptiness in the top of my breasts without going too big. Nobody would know I have had anything done.
Have you ever had any other cosmetic work done?
Yes, anti wrinkle injections.
What appealed most about this procedure?
A simple procedure without surgery. No downtime. Not obvious to other people but looks better.
How long does it take?
Forty minutes.
What is involved?
I sucked on gas, had a local anaesthetic and felt some pushing and pulling. At the end I got up, looked in the mirror and, "Oh my God, they look great!"
What is the recovery time?
I was back to work the same day.
Did it work?
Yes, straight away. It was fantastic.
Did it hurt?
Some pushing and pulling, which was a little bit uncomfortable. No real pain.
Any side effects?
No, not really. There was a dull ache for 24 hours after.
Would you do it again?
In a heartbeat.

Macrolane Trial

For a truly uplifting experience, go up a cup size in an hour without surgery, writes Becky Barker.
It could only be a matter of months before women are queuing up for a walk-in, walk-out, go-back-to-work boob job in Australia. Dr Anoop Rastogi, who has a practice in Double Bay, started clinical trials on three patients with the new, injectable, gel-like filler Macrolane last month and says the patients love the look.
"There was a little bit of swelling, they took mild painkillers for a couple of days and were back at work the same day," says Dr Rastogi, who administered the treatment at 40-minute appointments where the subjects received a local anaesthetic under light sedation.

Dr Rastogi says the technique has since been refined and he has 100 patients on a waiting list if Australia's regulator for medical drugs and devices, the Therapeutic Goods Administration, gives Macrolane the go-ahead. It is said to suit people who are averse to surgery and scarring and want a subtle change from A to B or B to C cup.
The new filler is a thicker form of Restylane, which has been used as a skin-plumper for 10 years.
Despite that, there are big concerns about the jab's safety, cost - expected to be about $5000 to $7000 - and longevity. The effects last about a year before needing a top-up.

Breast implants changed my life

Jacqueline Achilles is a fitness instructor and part-time model who wants a career in fashion.

"I was always told my breast size wasn't ideal," says part-time model Jacqueline Achilles. "And when I went shopping I felt limited about what fashionable clothes I could wear."


So Jacqueline had breast implants and now, she says, "I feel more confident and feminine."


Sydney cosmetic surgeon Dr Anoop Rastogi says that while Jacqueline had beautiful shoulders and nice hips, she lacked an hourglass figure as her breasts were small.


The aim was to create breasts to balance and sculpt her figure, which would allow her to pursue her modelling career.


Jacqueline had tubular-shaped breasts with a short distance between the nipple and the crease under the breast. 



Because of her natural breast shape, Dr Rastogi chose a very specialised anatomical implant (tear-drop shaped) so he could lift the nipple position and correct the tubular shape.

The breasts slope gently from Jacqueline's shoulders to her nipples, then arc fully to the creases under her breasts.

The width of the implants created cleavage, and fullness at the sides made a contrast with Jacqueline's waist, producing the desired                       hourglass figure.

Five rules to remember:
  1. Patients really need to do their research when choosing a surgeon. How attractive the breasts will look has a lot to do with the surgeon's skill.
  2. When looking for a surgeon, check the Australasian College of Cosmetic Surgery website for advice. www.accs.org.au
  3. Look at before and after photos to determine the shape you want.
  4. Have in mind the cup size you would like to be, but also be guided by the surgeon's knowledge.
  5. The most often-used implants in Australia are cohesive silicon gel, they give the most natural look and feel. Saline implants can wrinkle and have a higher failure rate.

My Artistic Endeavour


I am a Breast Augmentation Surgeon known for my outstanding surgical results creating very natural breasts.

When the surgeon desires it to be, and has the skills to allow it to be. For as long as the surgeon sees the surgery merely as a procedure where an implant is placed into a breast to make it bigger, it can be nothing more. 
However with the artistic skill to visualise a breast sculpture with all its natural beauty, balance and proportion, and a desire to utilise all ones surgical skill, knowledge and experience to create the beauty that one has envisaged then ………. An operating theatre becomes a studio, a surgeon becomes an artist and surgery becomes an artform.

The Final Word on Facelift Post Operative Instruction


It is normal to have some degree of facial numbness post-operatively. 

This may sometimes continue for several weeks following surgery Facelift surgery (Meloplasty). 

Some patients may start to experience a tingling sensations and even small sharp pains one to two weeks after the procedure. This is quite normal and is due to the nerves starting to regenerate and will resolve itself. 

That said, I always encourage my patients to contact me with any concerns.

DAY 5-10.

Any sutures that require removal are removed on day 14.

Bruising and swelling may differ from person to person but generally 5-10 days is the optimal period for this to subside. 

It is possible though that these symptoms may remain for up to 4 weeks.


Make–up may be worn after the sutures are removed, but only on the actual suture lines when these have completely healed. 

Chemicals may also be applied to the hair 4-5 weeks later.

DAY 2.

Discontinue using ice packs.
Hirudoid ointment may be applied to bruised areas, do not apply to broken skin (ie. suture line).
This ointment may be obtained by the chemist.


DAY 3-4.
You will have a 4 day follow up in our rooms.
You may wash your hair with an antibacterial shampoo (phisohex). Ensure suture line is dry after showering.

My face Lift post operative instructions - Step by step

DAY 1.

Patients usually experience only minimal discomfort following facelift surgery due to good pain management, and gentle surgical techniques.

You may apply ice packs to both sides of the face to help reduce swelling. Do not apply ice packs directly onto the skin, wrap them in a towel, tea towel etc.

You will have a pressure dressing or face form support in place for 24hrs and then a lighter support dressing for a further 24hrs after that.


Generally a semi-upright sleeping position (2-3 pillows) is preferable for at least the first week.

The three Facelift areas

The upper face consists of forehead, eyebrow and eyelid area. Dropping brows and deep furrows in the forehead area can be corrected by an incision behind the hairline followed by elevation of the skin, which is then pulled tight and excess trimmed away. This tightens the forehead and widens the eye opening, muscles can be freed to release wrinkles and frown lines. Only a fine scar is left behind the hairline.

The mid face involves eyelids, nose and cheeks, down to the jaw line. The lax skin and muscle in this area produce deep wrinkles and folds running down from the nose and jowl formation. An incision extends from the temple downwards over the small cartilage bump in front of the ear to the earlobe. The skin is then freed and gently pulled back to give a youthful appearance.

An SMAS procedure can be performed which combines tightening of sagging facial muscles with the overlying skin which gives a more natural, longer lasting appearance. The scars are again hidden in the temple hair around the ear.

The lower face and neck ageing problems include formation of jowls (‘turkey neck’), double chins and fatty deposits in the neck area. The incision begins around the earlobe and extends behind the ear and in the hairline without being evident even with the hair pulled back. Liposuction to the neck and jowls is possible to remove the fatty deposits.

Breast Augmentation - Placement and Incision


The most common placement of the implant that Dr Rastogi uses is subfascial, which is between the fascia (the outer lining of the muscle) and the muscle. Dr Rastogi feels that this placement gives the best result and is less painful than underneath or above the muscle.

After the implants are in place, the incision is closed with stitches which are dissolving, and a paper tape is placed over your incision lines. You will be told to avoid heavy lifting and bouncing for a period of 6 weeks, after that time you will be able to return to normal activities. You will be advised to watch for any signs of infection, bleeding or bruising. Signs of infection are pain, redness, swelling and fever. Infection is an infrequent problem, which needs immediate attention with use of antibiotics.

Breast Augmentation - The facts


Breast Augmentation procedures are performed in a hospital and it is advisable to discuss with your Doctor and anaesthetist, the type of anaesthesia, whether general (fully asleep), local (awake with numbing of the area) or a combination of laical with mild sedation (partially awake). The risks of each type should be fully explained and understood.

There are three types of incisions used for inserting breast implants:

  1. An inframammary incision is made under the breast at the fold or crease line. This is the most common incision.
  1. A periareolar incision is made around the edge of the areola (the darker skin)

  1. An axillary incision is made in one of the crease lines of the armpit.

Dr Rastogi prefers to use the inframammary incision, as he feels this gives the best access to the breast tissue.

Dr Rastogi uses silicone cohesive gel implants that have the consistency of ‘turkish delight’.

A liposculpture was performed with emphasis on superficial undermining to ensure skin retraction. Liposuction Information


Several years ago a new procedure, liposuction surgery, was perfected by a French physician. The procedure allows both men and women to improve the contours of their bodies through this relatively simple and safe technique.

Liposuction surgery involves the removal of fat cells from the body and is designed for those who have specific areas of localised fat deposits and who have tried unsuccessfully to eliminate them through diet, exercise and weight loss.

While the procedure is not designed to correct general obesity, any areas where excess fat deposits have accumulated can be treated, these include the chin, neck, jowls, cheeks, arms, inner and outer thighs, buttocks, knees, hips and abdomen. Men with gynaecomastia (enlarged breasts) can also benefit from this procedure.

Men and women in good physical condition with good skin elasticity are the best candidates for this surgery.
